oudiz said that all u.s. and european guidelines essentially make the same recommendations regarding treatment of pulmonary arterial hypertension: if a patient is stable and does not have high-risk features - like heart failure - then therapy should be started with an oral drug, but if a patient is very sick and in the hospital, then therapy should be started with a prostacyclin with a bias toward an iv prostanoid.
